6. Transfer Order if available
7. No. Enquiry, No Demand, No Audit Para, No Punishment Certificate.
8. Appointment Order copy
9. ID Card Copy
10. NTS Copy
11. Medical Certificate copy
12. Salary Slip
13. Matric, Inter, B.A/B.SC, MA/M.SC, B.Ed/M.Ed, copies
14. Service Book Copy
15. School Result last 03 Year
16. Affidavit, signed by DDO’s RS. 50 at least
