Subject:- VISIT REPORT
The undersigned visited Govt. Boys High School Kot Samaba RYKhan on 17.11.2016 at 02:00 pm in a surprise way.
Following are the observations.
| 1. | Syed Muhammad Azam Gilani SSS | Attendance was marked (Principal claimed that he has sent him). |
| 2. | Muhammad Siddique Bosan SSS, | Attendance was marked but he was found absent. |
| 3. | Qazi Fasih-ud-Din SSS, | Attendance was marked but he was found absent. |
| 4. | Ch. Muhammad Siddique SSS, | Attendance was marked but he was found absent. |
| 5. | Muhammad Sharif SS, | Attendance was marked, but he was found absent. |
| 6. | Abdul Salam Gujjar SS, | Attendance was marked, he was found absent, but he came back at 02:30 pm. |
| 7. | Qayyum Akhtar SS, | Attendance was marked, he was found absent, but he came back at 02:49pm. |
| 8. | Abdul Razzaq SST, | Attendance was marked but he was found absent. |
| 9. | Muhammad Sultan Hashmi SST, | Attendance was marked, he was found absent, but he came back at 02:35 pm. |
| 10. | Ghulam Raza EST, | Attendance was marked, he was found absent, but he came back at 02:30 pm. |
| 11. | Haji Riaz Hussain EST, | Attendance was marked, he was found absent, but he came back at 02:35 pm. |

Note. There was nothing written on movement register about non availability of teachers during school hours.
School Condition
- There was no dairy found in the school
- Cleanliness was very poor.
- Windows Glass pans were broken and no attention was paid to repair those.
- Wash Basins and Wash rooms were very dirty.
- Condition of plants and greenery was pathetic.
The Principal is hereby directed to be present at this office and also direct the above noted teaching and non teaching staff to attend this office on 18-11-2016 at 03:00 pm along with supporting documents if any in support of your defense, to explain the reason without any sanctioned / allowed of causal leave of their absence on 17.11.2016.
